The Division of State Group Insurance has created a short survey for employees who have opted out of health insurance coverage. This survey launches on Monday, Dec. 3, 2018, and is available through Friday, Dec. 14. A reminder email will be sent on Monday, Dec. 10.

The survey is hosted by Survey Monkey and is completely optional. Participation is voluntary, and responses are anonymous.
